Designed for patients without coverage, our in-office plan gives you access to quality preventive care and significant savings on all other treatment – with no annual maximums, deductibles, or waiting periods.

Payment Options
• Annual Payment: One-time annual payment at a discounted rate
• Monthly Payment: Available at regular pricing. A one-time $119 enrollment fee applies to all monthly plans
Exclusions and Limitations
• Membership is not insurance and cannot be used with any dental insurance plans or discount programs
• Plan benefits apply only to services provided at Premier Dental
• Membership discounts do not apply to:
Referrals to specialists or outside providers
• Fluoride is only included in the Child Plan (age 12 and under) and is exempt from the 20% off
• Membership benefits are non-transferable and non-refundable
• Unused services do not roll over to the next year
• Membership must be active and current to access benefits
